Abstract
Paroxysmal cold hemoglobinuria (PCH) is one of the rare autoimmune hemolytic syndromes. It is characterized by acute intermittent hemolysis, hemoglobinuria, abdominal pain and periodic attacks of fever, followed by local or general exposure to cold, and positive Donath-Landsteiner test. PCH shows the Donath-Landsteiner antibody (D-L antibody) that is a complement binding immunoglobin G, anti-P biphasic hemolysin, causing significant red cell destruction.
We report a case of PCH following mycoplasma infection which was confirmed by increased mycoplasma antibody titer, cold agglutinin and D-L antibody. The patient complained about dry cough, headache, high fever, jaundice and hematuria. In the antibody identification test the reactions with all panel cells were positive.
